OC Chiro respectfully requests that all new patients fill out new patient paperwork and submit it through our webpage online, before you schedule an appointment with the doctor. The process is quick and easy. (Please be advised that you cannot fill out New patient forms using an IPad, this is something that is being worked on by IT and we hope to have resolved in the near future)
Please CLICK HERE to fill out your new patient paperwork. Once complete, hit the submit button at the bottom of the paperwork. Once you submit your paperwork you will be taken to a page that will allow you to schedule your new patient appointment. If for some reason you are not sent to the appointment scheduling page please text our office at 714-895-9355 to confirm that we received your new patient paperwork.
Your doctor will review your submitted information and reach out to you via (e-mail, text or call) before your visit date and time. This is a great opportunity to get acquainted with your doctor, and to ask any questions you may have.
