To make a New Patient appointment with our office, go to the New Patient Center tab on our webpage, Click on, online forms and fill out the following forms: New Patient health history form, New patient questionnaire and the New patient informed consent to treat form. You will be prompted to submit these forms to our office via our webpage once the forms are complete.
These forms must be submitted to us using our webpage in order for you to continue the appointment making process. Once you have submitted your forms you will be provided with a link to click on that will take you to our online automated scheduler where you can make your New patient appointment.
Prior to your first appointment your doctor will have reached out to you (Via e-mail or phone call) , to introduce themselves, as well as review your new patient paper work. This is a good opportunity to get acquainted with your doctor, and to ask any questions you may have.
On the day/evening of your first appointment your doctor will personally greet you, review your paperwork and then perform a detailed consultation. He will address any questions or concerns you might have prior to your adjustment.

Next, the doctor will perform a detailed examination. Your Exam will include necessary Chiropractic, Orthopedic and Neurological evaluation.
If X-Ray and / or Laboratory procedures are warranted, it will be discussed at this time.
Note: OC Chiro has no on-site X-Ray capabilities.
Upon completion of the examination process- a Report of Findings will be given. The outcome of your evaluation will be discussed. A diagnosis (if possible) will be provided. A treatment dosage prescribed to remedy the diagnosis will be outlined.

If you are comfortable, and at ease, with the diagnosis and dosage required to remedy the condition, you First Treatment will begin. No treatment will commence without your consent.
Treatment consists of..... Moist heat (hydroculator packs) , manipulation (adjustments) - and post treatment recommendations.

Upon completion of your first visit, contact info for your doctor will be provided, along with a review of your treatment dosage and accompanying future visit dates (if any) .
Any recommendations for referrals to outside Specialty providers and / or the medical profession may be discussed at this time.
